1) Please follow the order form all the way to the payment method page with the total amount including shipping and handling.

2) Please print it out and mail with your payment to the address at the bottom of the page. Checks (US band issued only), money orders, official checks are welcome. All funds in US dollars please. If you would like to pay with Western Union please email me first.
